disabled form

    I have a form that is disabled and I can not fix it.  I have cloned it to and nothing.  Please take a look at it.  Its the  Custom Check Information Form and the Clone of Custom Check Information Form.

    I have just reviewed a couple of your forms and was able to open them up with no problems.
