on our Wholesale Account Request form, we are receiving the notification emails to "email@example.com" every time. For example, we did not receive the submission below for some reason but the others came through earlier:2016-05-19 10:46:10IP188.8.131.52

I checked our mail logs for firstname.lastname@example.org and it appears that the email notifications were sent successfully to that address.
Can you please check your spambox to see if the email notifications were sent there? I see that you are currently using your own address as the sender email. If you are still not receiving the emails, please try changing the sender email to email@example.com instead.
You can also check your account's email history to see the emails that were sent to your address. Simply follow this guide: How to view all your form Email History
