Form with conditions is glitchy.

Hello,

This form is set up with conditional logic so that only four questions appear at one time, starting with the first four (all start with the number "1", the next four start with the number "2", etc.); however it is glitchy. Sometimes other questions are appearing from the start, when they should not be (I believe it is a glitch because different questions appear every time I open it). 

Your help fixing this is appreciated.
