None of my forms are showing and it's stating "Form not found. This form is disabled." I suspect this happened when i tried to create a form that gathered a username and password for a Wordpress site. I have revised my form and instead ask for them to put any information regarding their site into a document to attach to the form. Is that allowed?

Yes, that is most likely the reason why your form account was suspended, it is stated in our terms that you cannot ask for login credentials, CC info or other type of critical data in your forms, I would suggest you to avoid using words such like password, username or similar, although you're not asking for that info our anti-phishing system will catalog the form as phishing, please take a look to our terms as well: http://www.jotform.com/terms/.
I have checked your forms and noticed that, as you say, the fields are not there, but please follow the advice from above, it will help you to avoid inconvenient because of your forms being disabled.
