    The published version of the form is different from the form designer version

    look at the link of teh form I have created (look nice for me ) and the result on my website after embed HTML code completely different

    I could replicate the same situation:

     

    It seems that you selected the source code publishing mode. This means that you also need to upload to your website the css stylesheets. More simpler, and if possible, you can instead use the iFrame publishing method:

    This is the recommended method as it also eliminates possible code conflicts that may occur.
