Form not working on our site

Hi,

One of our jotforms (id=61295138482965) is not working on the booking page of our website: http://barkbud.com/get-started/. The form is embedded on our wp site using the wordpress plugin.

We did not make any changes to our website. Would you be able to help us figure out what may be going wrong?
