    Could you please tell us if that was shown to you instead of the form or once the form was submitted?

    I have tried submitting it without any fields filled out and it went through without any issues.

    Looking at the error you have in the screenshot:

    "alert('Favor colocar seu nome, campo obrigatorio.')"; echo"" it does not seem as the error that would be sent from our servers. Still, do let us know if you are experiencing the same issue at this time and if so we would be happy to give it another look.

    Just let us know what to do to recreate it on our side.
