I see that your account username peterCuocolo is on "Free" plan. But I see that forms are shared to you from an account with username pcuocolo, that one is on Premium plan. You can log in to that account and check the Billing->Invoice section to see the charges made. Here's a guide on how to check your invoices: https://www.jotform.com/help/329-How-to-Check-Invoices
