I created and published a form in jotform. I reopened account it said I didn't have any forms. How do I get the website embed code?

    Note:  My account said I had almost run out of forms and would have to subscribe soon.  How can I be running out of forms if I don't even have the form I created?

    This is the form I am looking for:

    I sent it to my website master who embedded it into my website (without the jotform embed code)  When she added it to the website the boxes for the person's name and telephone number did not look the same as my view when it I created it.  The phone number especially is unacceptable.  The area code is on one line and the phone number is on a second line and the words area code do not align under the area code input box.

    I'm nto sure where did you see that message. But that form is under your account with username: joanintegritycmgcom.  You can find it here, when you log in.

    You're almost out of views though:
