Creating a form from a PDF file

I've been asked by my computer club to add a signup form we already have that is in pdf format to the website that can be printed out by a member and was told Jotform could do the job.  We are already using Jotform.  I signed into the account but I can't find a way to do this.

We have the fields that would allow you to create the form. However, the design may look different. I've created the form with just the default template, here it is:
