Why some fields are not appearing in PDF report?

    My form was working fine until I added a Configurable List widget. Upon testing the form afterward, some of the fields are missing from the pdf report.

    Also I am not receiving email notification although autoresponder is working fine and showing all the fields as well.

    Any advice?

    First I can see you have enabled the option to hide empty fields in PDF report, this is the reason why you can notice some fields are not appearing: 

    I can see you have also customized your PDF report and you have removed some fields that although these fields have info submitted on the they are not displaying in the PDF report, I would suggest you to restore the PDF report, to do it follow the steps below. 

    On the submissions page, click on the option PDF -> Customize PDF:

    Now click on the Reset button, this will restore the PDF layout to default, download the email and you should get all the fields in the PDF report:

    Resetting it fixed the pdf.
