I cloned your form and made some tests there. I cannot seem to replicate the issue on the cloned form. All of the submissions were displayed correctly.
Can you please try removing the previous Google Spreadsheet integration in your form? After removing the integration, clear your form cache by following this guide: How to Clear Your Form Cache?, then re-integrate the form with a new Google Spreadsheet.
