Some of my radio buttons are not able to be clicked

    In my family grid widget, I have some radio buttons in three columns.  For the first and third columns, when someone tries to complete the form, it defaults to select "No" and the user can't select "Yes".  Only the second column is allowing the "Yes" answer.  Why do some of the radio buttons appear "locked" and how can I fix them?

    This is because of the custom CSS codes that aligns the radio button into the same line. The CSS made the first radio button to be placed behind the second radio button. I've fixed it for you by adjusting the CSS codes.
