My notification email shows as being sent from =?UTF-8?B??= <support@fleetwoodonsite.com>. The friendly name should not be "=?UTF-8?B??=" but I can't seem to change it. Also, the one field I have in the notification is email, but instead of the actual email address that was submitted, it is showing the word "email". I double-checked and I have the email field in there, not the word "email" so I don't know why it's not working. Also, when I have the email notification sent to me at steve@fltwood.com it does not work, but if I use another email address, it does work. What do you think?

Your email address steve@fltwood.com appears to have been added to the bounce list, which may be why you are not receiving notification emails there. I have now removed the email address from the bounce list so if you choose to use this as your recipient email in the notification, you should receive the notifications successfully.
You can also check on your end if an address is in the bounce list by following this guide: How to Remove Your Email Address from Bounce List
As for the email in the notification, I checked the most recent submissions and they appear to be displayed properly now, so no further action is required.
I also checked the "sent from" of the email notification and it seems to be displayed correctly.
If you are still encountering this issue on your end, please change the sender email from your own email into noreply@jotform.com.

Maybe because I was testing multiple times with my email address? Anyway, thanks for that info. However, my last remaining issue is the “sent from” name for notifications to me when a new person submits the form. It is still coming from =?UTF-8?B??= support@fleetwoodonsite.com where I would like the “friendly” name be something other than =?UTF-8?B??=. See latest submission attached and you’ll see what I mean.

You are most welcome. Your mail server may have been unavailable at the time the notification was being sent, or it may have rejected the notification from jotform, and thus the email address was added to the bounce list.
As for the UTF-8 issue, I checked the latest submission and I cannot seem to see the issue you are referring to. I do not see the UTF-8 in the "From" section of the email. You can also check the email from your email history. Simply follow this guide: How to View All Your Form Email History
May I ask what email client are you using to view the email? The email might not be read properly by the client, and so you get the UTF-8 error instead of the correct text. Can you please send us a screenshot of the issue? Here's how: How to add screenshots/images to questions to the support forum?

In JotForm, Notification email details, Recipients tab, the first option is “Sender Name” with a dropdown list. The only option is JotForm, and no option to add anything else, so I can can’t make it a “friendly name” of my choice. So, it comes with no name at all, and apparently Outlook 2013 interprets that as “=?UTF-8?B??=”.

Since there is no Name field in the form, the default Sender Name of the notification is set to "Jotform". If you want, you can have a different Sender Name by adding a hidden name field in your form.
1. Add a textbox field and set it as your Name field. Add a default value to the field.
2. Then set the Name field to hidden by clicking on the gear icon and selecting Hide Field.
3. Then you can now select the Name field as the Sender Name
The email you receive should now have the default Name.
If you do not want to see the Name in the body of the email, you can delete the row for the Name field.
If the issue still persists after, please let us know.
