Is there a template to create a quiz that once the quiz is taken, it will give you the answers if you submit your email address?

I want to make a quiz that patients can take. Once the patient has completed the quiz, I want it set up to where they can retrieve their answers or score by submitting their email.

Yes, we have an app which you can set your form to become a quiz form. Simply visit https://apps.jotform.com/app/quiz_form and start using the app. It should start by letting you select the form that you want to become the quiz form and just complete the wizard.
