Our forms can not be found and account is suspended

Asked by TropicalBlueTravel on May 24, 2016 at 09:48 AM

I keep getting this error  message on my forms.  The internet site reports that the item you requested could not be found, (http/1.0404)  I am not sure when the error started.  I do know it was working a few days ago.  The two forms that I know it is not working on are as follows

When I go in and try to find the forms I am now received a additional error message

Due to <a target="_blank" href="https://www.jotform.com/terms"><b><u>terms of use</u>,/b></a>violation, your account has been suspended.  If you think this is a mistake please <a target="_blank" href="http;//www.interlogy.com/contact.html?jotform=yes" target=blank:><b><u>contact us</u></b></a> to resolve this issure.

I am not sure what I did or how to fix it.  Please advise.

The reason why your account is blocked from access is because the forms as well as the account are suspended.

The reason for suspension is the breaking of our terms of use and asking for credit card details.

    Please check the following:

    http://www.jotform.com/terms/

Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose is found to be deceptively obtaining: sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.

As such it is required for us to delete your forms collecting the same data.

The only way that is allowed to collect credit card details is by using payment processing tools.

Setting Up Your First Order Form

The only option at this time is to remove the fields that are used to collect the same details, or the entire forms. If only fields are removed you would keep the forms, while in the same time it would not break our terms of use.

I've spent the several days working on four different forms, so today I look under previous questions, said I was accidentally disabled.  I just logged in and it says I have no forms.  Please tell me this will be corrected, or I may have to shoot myself.

When testing the import feature, we accidentally imported a login from an account we own. It was not intentional since we are novices at using the forms. Would you be able to restore our forms and account?

    Thank you.
