I have 3 fields that aren't showing up on the form for my customers (or even in the preview). They are not hidden and there are no conditions on the fields. What other settings can I look for to activate those fields? They also don't show when I go into edit submissions on the form.

I have checked your account and you have only this form http://www.jotform.us/form/61435222484149, so I assume that that is the form you are working on.
I have inspected your form and all fields that are shown inside of form builder are also shown on your live form. Were you able to resolve your issue?
If you are still unable to view all form fields, please tell us which fields you are unable to see on your live form. If possible please provide us a screenshot. In case that you are working on some other form, please provide us ID of that form.
I also noticed that you have conditions inside of your form for fields that are deleted from the form. I would suggest you to delete those conditions.

Interesting. I deleted the fields that were causing problems and I added new ones that are now working. I used a template to build the form. I knew that I didn't put any conditions on it buy maybe the form had some already built in from the template? It must have been the issue since there are 3 conditions and they are all relating to deleted fields!
At any rate, I think I got the fields to do what I needed them to do by deleting them. I'll be sure to delete the conditions as well.

I am glad to hear that you were able to resolve your issue. It is possible that those fields were not shown because of the condition that was set inside of the form.
