Why is my form not sending?

This form has been in place for some time - https://form.jotformeu.com/32563125974357 - but it has stopped sending at some time in April, by the looks of things.  After pressing submit, nothing happens - no confirmation, no email.  This is the page it is linked from: http://greenbankpreschool.org/how-to-apply/ .  I've checked the form, and can't see any reason it is not progressing.  Please can you take a look?
