All forms are disabled?

    Seemingly for no reason, all of my forms are now disabled. I was right in the middle of working with one. I know I am not over my submission limit as it is used very infrequently. Is there a known outage or server issue?

    The given form is asking for a username and password. Well, the fields are used in a condition to show the other fields, but similar forms or forms that would ask this information are considered as phishing which is against our Terms of Use (http://www.jotform.com/terms/). 

    I've reactivated your account, but I would suggest editing the fields and change the labels not say these kinds of terms. You can say "Authentication Code".

    Thank you. It was reactived but now is down again? I made the  change as suggested.
