Form fields suddenly not showing on the form

One of our forms has disappeared. It is still listed under MY FORMS but when we click on the form to edit, it says loading and then its blank and gray. Please advise. We would like to revert back to previous version if possible. Thanks.

I would suggest you try to check the revision history of your form and revert back to the time where the form or fields are still showing.
