I have a jotform with a uniquie ID for each record and use Zapier to send results to a mysql database. I have a duplicate of that form, but have changed some layout items and use Zapier to send results to the same mysql database as the original form. This isn't working because a unique ID in form two is duplicates in the mysql database. I would like to additionally clone the form multiple times with different layouts and continue to send results to the same mysql db. Any suggestion how I can resolve this?

I would suggest utilizing the Prefix option in the Unique ID widget:
You can say Form1 then Form2 on the second and so on. This would keep every submissions unique and won't create any error in your MySQL Database.
