The forms on our website have suddenly stopped working

All of the forms on our website that we've built with Jotform have suddenly stopped working. We haven't changed anything. Not sure why this is happening. Please advice asap. Thank you

Your account is currently active. I checked your webpage and I can see the form is showing properly. I tested it using Chrome, Firefox and Explorer.  Here's a screenshot:

It is working now because I did a little research and it informed me that our email got put on the bounce list. Not sure what had caused this but as soon as I removed it we started getting the email notifications once again. This was after we took the time to rebuild all of our forms thinking they weren't working properly. Im glad the issue has been resolved but we did have quite a few unhappy clients that had to resend their information to us several times.
