Is it OK to ask for a name of person making reservation on our payment form?

I created a on-line deposit form and jotform revised some of the fields (phishing activity) .  I would like to add the name of the person who is make a reservation. would this be violating my account w/jotform? 

would it be all right to add the field: "name of person making reservation"

No, you can ask for the name. There should be no issues with the same.
