Formulario desaparece

Buenas trades, tengo un problema con un formulario que monté en una caja de iuminación con se despliega con un boton de acción, el problema es el siguiente: cuando llamo a la acción aparece el formulario, pero inmediatamente desaparece y se baja en la pantalla y tengo que hacer scrolling para verlo y llenarlo, como comprenderan no es conveniente ya que es un formulario que me sirve para hacer una base de datos de clientes potenciales.
