Why does my form come up as disabled when I try to load it onto my weebly website?

We apologize for the inconvenient this may have caused. 

I have checked your account and I can see it is currently suspended, this happens when our anti-phishing system finds that you're asking for information that is not allowed at JotForm, then your forms are disabled and your account suspended. 

However, I have checked your forms and I can see they are clear, I have activated your account now and you should be able to load your forms as normal, I would like to share you the link to our terms so you will avoid to ask for info that is not allowed and avoid to get your account suspended due to this reason: http://www.jotform.com/terms/.
