Despite of my form is ACTIVATED, appears Form not found.
I have been used it whitout problems recently.

I have checked your forms and noticed you seem to ask for a pass code or something similar, although you would not asking for a pass code or password, our anti-phishing filter detects this as phishing and then proceed to suspend the account.
I had to delete this field in order to activate your account, please read our terms this is stated there: http://www.jotform.com/terms/
I would suggest you to avoid using words such as username, pass, password, code or something that may indicate you're asking for info not allowed at JotForm, although you're not using your forms as phishing they can be disabled.
