Eu criei dois formularios aqui na hora de visualizar o mesmo é bloqueado.
Oque preciso fazer para isto não acontecer?

I checked your form and it looks to be within our terms of use. I reactivated your account and your form should be live once again. Take care requesting any sort of banking information. If you request information further than simple account numbers for transfers and things fo that nature, your account may be locked again.
