Formulario Bloqueado

Por favor Porque Meus formularios were bloqueados, DEPOIS de Editar NÃO Consigo visualizar Como ficou, aguado Uma Solução.

Gostaria de ter minha conta desbloqueada assim que puder acessar irei apagar todos os formularios que foram bloqueados e fazer outros que estao dentro das normais do site

@idcp I have removed all the forms from your account that were in violation of our terms of use and activated your account.  Please refrain from requesting usernames and/or passwords through your forms as it will result in your account being suspended once again.
