I cannot get rid of the past information when I go to a ‘new’ form. I’ve tried it in both Chrome and Safari. Each time I try to get a fresh form, all of the prior information reappears.

It seems that your browser is saving the information in its cache. Jotform also enables you to save information in your browser so you may continue it later.
To disable this feature, enter the PREFERENCE wizard and enter the ADVANCE TAB. Just disable the feature CONTINUE FORMS LATER
