I have add the suggested CSS code to my form and also tried the Widget for mobile phones to be able to view my form on the phone properly.Asked by PatWynd on May 27, 2016 at 05:45 PM

I checked your form and was able to see what you're talking about, I would suggest you to remove all the code you currently have in your form and inject the code in this link: http://pastiebin.com/embed/5748cef978fce
All that code will help you to display your form properly, in the case of the matrix fields, when using radio buttons and when they are a lot, this can be a bit hard to achieve since the matrix field uses a table to work and tables cannot be easily mobile responsive, but this code should help you, here is the link to my cloned form where I applied this code: https://form.jotform.com/61477026225958
