like so many other users in your forum... my form has been disabled. please re-enable my form asap. thank you.

Your account appears to have been autosuspended by our phishing detector due to suspected phishing (collecting username and password, credit card info). However, I checked your forms and they do not seem to violate jotform's terms. I have now removed the suspension on your account. You should be able to view your forms without any issue.
