Is there a way to create a custom URL without having your jotform username shown as well in the link?

Unfortunately no. Please be noted that if you create custom URL of your JotForm, it will always include your JotForm URL.

I would suggest you to use your own domain to access your form instead. You can take advantage of our custom domain feature instead of custom URL and that should solve your problem. The follwoing guide should help you:  https://www.jotform.com/help/144-How-to-use-Custom-Domains-with-Jotform
