Having trouble with the form on facebook in mobile view

    When I test this view from an IPad mini on an account I use to test things, when I click the sign up button, it appears to be blank unless I scroll down then it is a bit jumbled.  

    I also have a tab on another page that I can not get to work on mobile at all.  The second page is https://www.facebook.com/YourAgentRocks/   3rd tab from the profile pic, 0 down workshop.
     

     

    Can you please tell me how to fix these two items.  Thank you very much!!!

    I clicked the Sign Up button in your FB page - https://www.facebook.com/0DownWorkshop/ - and it redirected me to the direct link of your form - https://form.jotform.com/61476114404146. Here are the screenshots of your form when I accessed it using iPad mini:
