How to allow users to search directly on the form?

    I'd like for my form entitled "Weekly Meeting form" to be able to collect names & phone numbers and then keep them in a database that is searchable.  I'd like to have the database list the names in alphabetical order if possible.  How can this be done?

    I have inspected your form and I saw that are using User Contributed Dropdown widget. This widget allows your form users to add an option to your drop down field, in your case name and phone number. However new name that is added will be placed at the bottom of existing names. It is not possible to list them alphabetically automatically. 

    To collect names and phone numbers you can also add Full Name and Phone fields to your form.

    You can also search for names that were entered inside of your form on Submission page of your form. Inside of this guide you can find how to view submissions:

    On submission page there is a textbox to enter information to be searched from the submissions received data. you can also list your names in alphabetical order.

    You can also export your data in excel report and then you can sort your data in alphabetical order.

    Thank you. That answer was interesting and I hadn't thought of doing it that way.  But, I need for the person completing the form to be able to do the search DIRECTLY FROM THE FORM.  How can I make that happen?

    Unfortunately it not possible to add search field inside of the form, so that you users can use it.

    Possible workaround is to use Auto Complete widget. You can place all the information (names) inside of Auto Complete widget. Inside of this guide you can find how to add widget to your form:

    However this widget will not allow your users to add new options. You will need to update your widget manually inside of Form Builder.
