Excel Report: Why a question is being omitted even when it's answered?

I just tried two submissions. 

The question "I'm a member of an anniversary chapter." is not on the Excel report. The second submission actual marked "yes" to this question.

This is because this field is not checked, it means that it is being omitted because you have set it like that, here is a screenshot about the current settings of your report: 

You need to check this option and the field should be shown in the report, do let us know if it is not.
