How to amend a current form

Hi,

I have tried to amend one of our jot forms and somehow have deleted the text boxes leaving me with only headings.  Can you please tell me how to get back these text boxes?

I can provide you with our user ID and password it needed.

You should be able to revert back to an earlier version of your form. Here's how: https://www.jotform.com/help/294-How-to-view-form-revision-history
