How to delete a field on a form?

    Hi,

     

    Can you please advise how I delete a field on a form? Somehow there are 3 places asking for email address. I also want to delete the address field. 

    I checked the revision history and there isn't an older version to revert to.

    I checked your form and I can confirm that there are three email fields on your form. To delete the duplicate email fields and the address field, simply click on the field that you want to delete and then click on the red X on the top right of that field.
