Form showing as disabled

Hi,

I need your help regarding to publishing the form. After I click publish button, the link is not working and I cannot reach the form. 

I would appreciate if you help me on this issue.

I checked your form and it was manually set to disabled:

While editing your form, under the preferences tab, change the status of your form from "Disabled" to "Enabled".  This will allow for it to be usable once again.
