How can I make a form with photos and add a response botton associate with the photo?

I would like build a form in order to do a "normative" study about how people evaluate different photos. I need to associate one photo with a response botton in order to randomise the presentation between subjects. It´s possible? This is an example:

A setup similar to yours would likely be the best solution.  Showing the image then having a scale rating field for the rating.  Unfortunately, there is no way to associate the images with a specific rating field.  Randomizing the order of the images would not be possible.
