    3. Is there any way to have a row without check boxes? On the page referenced below, I'd like to group the row descriptions under a few different headers...any way to do this?

    I am not sure if I understood you correctly. Do you want to hide all columns and show only rows?

    You can hide columns with CSS code. First remove all Columns from matrix table (only one will be shown).

    Then Inject this CSS code to your form:

    #id_5 th.form-matrix-column-headers.form-matrix-column_0 {

        display: none;

    #id_5 .form-matrix-values {

        display: none;

    Green part of the code is ID of matrix table. You need to add ID of matrix table to which you want that this code is applied. Inside of this guide you can find how to find field IDs and names:

    And inside of this guide you can find how to Inject Custom CSS codes to your form:

    If I did not understood you question correctly, please explain in more details what you are trying to achieve.
