I did some forms and when i will try to use, the message that my account was disable appear

Your account is now re-activated. Your forms should be working fine now.
It was auto suspended because one of your form is asking for email and password fields. Please note that asking information like login details (email/username and password field combination) is prohibited. This violates our terms. We have lifted the suspension in your account but we have deleted the password field in your form. Please refrain from recreating this field to avoid account suspension.
