hello. I would like to use this format with the radio buttons, but with option to select multiple buttons. Can that be done?
Currently I am using this payment form and would like the electric and website to be options under each event chosen.

I apologize the mixup I original had since I didn't see this thread right away so I fixed that thread follow-up question you had and connected it to this one instead.
Anyways, moving into your issue here if I understand you correctly it seems you wish to format your radio buttons to allow multiple options?
Radio Buttons are intended to only allow one option instead of multiple whereas for multiple it is recommended to use Checkbox Buttons instead which I see you have already. The only way to change this is by updating it in the form's full source code but then you'd have to embed that.
The reason you have checkboxes and the other thread you followed up has radios is because that thread user was using subscriptions as products which shows up as radios.
Please see https://www.jotform.com/help/49-Order-Form-Types to get a better understanding.

thank you for your quick response.
But I may need something other than what that shows (order form types)
I have multiple events, and each event has optional features that can be paid for.
I have attached an image of what I need.
thank you

Ah okay thank you for the additional details about what you are trying to create.
We actually have a guide found here https://www.jotform.com/help/178-How-to-Have-Sub-Category-or-Sub-Labels-on-an-Order-Form that walks you through the necessary steps to make that type of payment form with options under them.

I have seen that, but I need the radio buttons (or check boxes) enabled to check more than one.

In order to create the list-like categories you have to format it with the created product options which is mentioned in that guide I shared with you above and that allows you to still be able to check multiple options since those are checkboxes anyway.
If you meant something else then unfortunately I'm not sure that I understand so I apologize if I have gotten confused at what you are trying to do.
However, the image you showed me of what you want is the same thing as the guide I provided which is what I thought you wanted? Is that wrong then?
