My wholesale application displays blank page when customer submits the form

Every time a person submits a wholesale form they are greeted with a blank page and not the thank you I have set up?

Please advise

I have tried submitting the form from the web page and see that the blank page is displayed. However, when I tried to submit the form using its direct link, I see that the Thank you page is displayed.

As I check your JotForm on the web page, I see that the form is embedded using Source code which may be conflicting with other scripts on the web page. Please try re-embedding the form using iframe embed code and see if that works. You may get the iframe embed code by following the guide below:
