The form is disabled

I can log into my account and I just successfully built a form, but whenever I click on it I get "Form not found: this form is disabled".

Sorry for the inconvenience. I have just reviewed your form LASX EMPLOYEE REMOVE CHECKLIST. It seems that our phishing application suspended your account in error. Once reviewed I did not see any violation of our terms and reactivated your account.
