I don't think the form works at all now: there are 2 fields for the email now but it's not possible to fill in the first field and as a consequence there is an error message, the second one works but it's used to verify the first one (see image attached).
Did your guys ever test it? -

The reason why you can't fill in the first textbox of the email field is because it is set as "read-only".
