When I print my application, the header field does not print. Also, my terms and conditions information does not print.

It prints before you press submit, but when I go to the applications and try to print the submissions, it just prints the information they completed, not the additional information.  Terms, Header, etc. 

You need to enable the headers and text option, please check this guide: http://www.jotform.com/help/305-How-to-include-Headers-Texts-and-Images-in-the-PDF-format-of-a-submission
