What are the requirements on our side if we are using Webhook?

Web-hooks?

     Whats required on our side pertaining to web hooks?  Can we create the fields in Jot form and export to build the tables in SQL Server and then connect via web-hooks?  Is there an export process for this?

Export HTML and CSS?

You need to have the SQL information (authentication) ready in order to use webhook. You need to create the tables in your SQL server before using webhook. 

We have a few apps that you can use to export your form submissions into various database format, please look at here: https://apps.jotform.com/search/export. You can give it a try and see if you can automatically create the tables on your SQL Server. Please choose the correct format.
