I have checked submissions of your form http://www.jotform.co/form/60543347480859 and I saw that some of the data are missing.
Considering that you are located in EU, I would suggest you to check the Europe Datacenter option on Data page of your account. Please try to check/enable this setting and see if this will fix the issue.

Then please try login to your account using https://eu.jotform.com/ and check your submissions.

It seems that there is an issue with the submissions displaying blank between 13-May-2016 to 23-May-2016. Our developers have fixed this issue and you should be now able to view all the form submissions normally. We are sorry for the inconvenience this may have caused.
