Hi - We have been using the starter version for quite some time - we have received submissions in the past, but right now are not showing any received. Wondering if there is an issue that we are missing.

If I understood you correctly, you are not receiving submitted data via email Notification. I have check your form and you have four email Notification with four different recipients. Email address of your account was inside of bounce list and because of that email Notifications were not send to your email address. Email Notifications were successfully sent to other three email addresses.
n7*********wy@g****.com is IN the bounce list
Reason(s) : smtp; 550-5.7.1 Unauthenticated email from aol.com is NOT
I have now removed it from our bounce list and it should be working fine now.
You can follow this guide on how to check and remove the email from bounce list.
Also you should contact your email provider and ask them to Whitelist JotMails IP Addresses.
Please test your form and check if you are receiving submissions now.
