Why my form is not showing on sizes smaller then 1440px!?

Hy peoples, i'm having trouble with form, it is visible only on desktop size and when it gets to 1440px it disappears! I've embedded it over muse and can't se what seems to be a problem, I've set it to be responsive but it isn't it just disappears! Help please!?

I tried to check the iFrame within your website where you embedded the form, and found out that the link to your Jotform form is missing.

It should be like this.

 
I edited the source link using the browser's tool. And the form is working fine, even when I adjusted the sizes smaller than 1440px.

I would suggest to double check the iFrame within your website.
